In Year 7<\/strong>, students are offered free vaccines for Diphtheria-tetanus-pertussis (dTpa)<\/strong> Papillomavirus (HPV)<\/strong> at Santa Sabina College on Tuesday 12 August 2025.<\/strong><\/p>\n\n\n\n

Vaccinations will only be provided if consent has been received. <\/strong><\/p>\n\n\n\n

How to provide consent:<\/p>\n\n\n\n

  1. To complete online consent for your child\u2019s school\u00a0vaccinations, visit:\u00a0https:\/\/nswhealth.service-now.com\/school<\/a><\/li>\n\n\n\n
  2. Follow the steps to log in to your existing ServiceNSW account. This is the same account you may already use to renew your driver\u2019s licence. If you don\u2019t have a\u00a0ServiceNSW<\/a>\u00a0account, refer to\u00a0ServiceNSW<\/a>\u00a0to create one.\u00a0<\/li>\n\n\n\n
  3. Update or confirm your personal details in ServiceNSW as required.\u00a0<\/li>\n\n\n\n
  4. Complete the School\u00a0Vaccination\u00a0Consent Form for your child\/ren. You will need to:\n

    1. Enter your child\u2019s personal details.\u00a0<\/li>\n\n\n\n
    2. Provide the Medicare card details for you and your child.\u00a0<\/li>\n\n\n\n
    3. Read the\u00a0linked Parent Information Sheet and privacy statement<\/a>.<\/li>\n\n\n\n
    4. Provide consent.<\/li>\n<\/ol>\n<\/li>\n<\/ol>\n\n\n\n

      If you or your child do not have a Medicare card, consent can still be provided by requesting a paper-based consent form directly from me.<\/p>\n\n\n\n
